2 /5 Lillar Burton: My mom Been there over two weeks. She had open heart surgery. The CNA take a long time to aide care. Company seemed understaffed. A lot of the employees are just talking at nurse stations. I like two nurses who really do their best. John or Adam is the name of one. The other great nurse is from Armenia, I forgot her name. They also are out of supplies.

1 /5 Connie Jo Collins: June 2023 - The most horrendous experience of my life. They refused to follow my surgeons orders regarding my medications. They told me I had to ask for my meds. (I was taking 4 different medicines). Even when I asked for medicine for pain, it would take 2, 3 even 6 hours before it was brought to me. Getting help to the restroom often took so long, I ended up soiling my bed. With a couple of exceptions, the nurses were rude, some incompetent. (Definitely, they were under staffed. **The physical therapy staff was GREAT !

1 /5 Mari M: My mom was placed in this facility to get strong enough to come home. While there, she got C Diff(they said she got it from the hospital, maybe true, maybe not). Needless to say, after she had made some big "messes" in her bathroom and her room, that clearly needed to be washed, disinfected etc it took them 3 days of me and my sisters asking them to take care of it for it to be cleaned. Then when given the meds for her Cdiff I had to follow up to make sure she was receiving it...on what would have been the 2nd day of her taking the medicine I had to call 2 different ppl/pharmacys to verify the dosing as this facility messed that up as well.(they gave her meds 2x on the first day, and wasnt going to give it to her for another 10 days. According to Dr Google and local resources, the medication is given 4x a day for 10 days). I told the nurse on hand she needed to contact the Dr ASAP to check the correct dosing(they did fix it, later that night). On 3 different occasions I had to ask them to change her sheets as they were "dirty." This was not my moms first time at a rehab facility, nor my having to deal with the ppl at a rehab facility.**Idk what my mom thought of the therapy staff, I had a good experience when talking with them pertaining to my moms care.** This will definitely be the last time she is at this facility if I have any say in where she goes in the future. Im thankful to say she is back home and starting to regain her strength.

5 /5 Stephanie Berry: I broke my leg and required surgery and you that I would not be able to recover well at home with four dogs. I inquired and found out that I could go to Salmon Creek Post Acute and Rehabilitation and spent two weeks there under their great care. The first couple days took some getting used to as I had never been someplace like this, and I was still a bit in shock Over my recent leg break, but I truly had the best experience there and ended up staying for the full two weeks that my insurance covered. I definitely felt the CNA‘s and nurses genuinely cared about me in my condition and they were quick to answer questions or bring me freshwater or help me in anyway needed. I especially appreciated the overnight nurses as it’s a bit rough to get through a night in pain and not in your own home and they were extremely compassionate. I couldn’t believe the top notch service that I received through physical therapy and occupational therapy and that was a complete game changer in my recovery journey. I was there over Christmas and participated in a couple of the activities and ended up becoming really close with the woman in the room next to me. It was an all-around great experience and I highly recommend Salmon Creek Post Acute and Rehabilitation for any meeting such care!

5 /5 Voyles Lisa “BeautifullyFlawed”: I have worked at Salmon Creek Post Acute for 1 year now. I have finally found a home here at SCPA, where I can work side by side with a team dedicated to caring for others. The therapy department is amazing! Unlike other places I have worked , there is a higher level of accountability, a mindset towards solutions and genuine support for staff and patients alike. I can honestly say if I had a loved one who needed skilled nursing, this is the place Id want them to be. Id recommend this facility to anyone in my field looking for a job.
